Layout: separate context menu from main menu for better accessibility #110

Currently both main menu (my list, my account, log out) and context menu (actions specific to current page like "create list") are grouped into one HTML navigation and heading. It seems to me that moving context menu into separate HTML navigation will make it more accessible for screen readers, providing separate landmark and heading for context menu.

This must be reconsidered while working on #116 where I want to merge both navigation menus into one line.
