Pursuant to board meeting 2016-03-07 I'm submitting a PR that codifies the "three months, two notices" policy set forth by the board way-way-way back in 2016.

"membership revoked" instead of "dememberized" for consistency with the rest of the text
"notices from the board or treasurer" since it's mostly the treasurer sending out the notices
"arrears" not "arears"
